Form 1 H&K Markes M1 Super 90

So, I’m starting to fill out paperwork on a SBS. The gun is a Benelli M1 Super 90, imported and marked Hecker & Koch (importer).

So, on field 4a, do I list both the manufacturer and importer? After all, it does ask for the manufacturer and/or the inporter... If I put both, how do I differentiate between the two on the form? Also, what addresses do I use? H&K has moved since the gun was marked (Sterling VA) and the manufacturer (Benelli) wasn’t stateside.

Also, since this is a trust, I assume I would fill form 23 out the same (in regard to manufacturer and/or importer).

Things were so much easier with my little Remington 870 Wingmaster....

I would say much like a CZ Scorpion, where CZ USA is the importer and Česká zbrojovka a.s is the manufacturer, I put Česká zbrojovka a.s on my Form 1 along with Czech Republic.

So I'd go with original manufacturer and Country.
