At our range, we have numerous holes in the wall... all from instructors of Federal Agencies. None from Haley though.

It looks like he was applying pressure with his forefinger on the trigger guard with his finger tip and it slipped off and popped into the trigger guard causing the discharge. The trigger was depressed by the part of his finger between the knuckle and the first joint. (Or possibly right at that joint.)
